Sponsors

Together, we make compassionate care possible.

2025 Annual Sponsor Pledge

Each year, Prairie Home Hospice & Community Care invites local businesses, individuals, and organizations to join us as year-round sponsors. Your partnership helps ensure that our patients and their families receive the quality care, comfort, and support they deserve—at no cost to them.

Annual sponsors make an extraordinary difference through financial support and community engagement. In return, our partners receive recognition through print and digital materials, VIP access to select events, and the knowledge that they are part of something meaningful.

Please feel free to contact us at 507-337-0080 with any questions related to our Annual Sponsor Pledge.

Meet our Annual Pledge Partners!

It is thanks to generous donors like these that we are able to continue to provide resources such as quality staff, extra therapy options for patients, and outreach services to our families.

What Do Your Donations Support?

Prairie Home Hospice & Community Care is deeply grateful for the generous support from individuals, businesses, and community organizations. Your donations help us provide compassionate end-of-life care, emotional support for families, and vital community outreach programs. Every contribution—large or small—helps ensure we can continue offering comfort, dignity, and hope to those who need it most.

How Do Donations Make a Difference?

Donations allow us to provide the level of personal care and attention that hospice patients and their families need, deserve, and have come to expect from us.

  • Our care teams are committed to being responsive to the needs of our patients. We have an RN on call 24/7 and if the RN is not able to take a call, it is our goal to return calls within 15 minutes whenever possible.
  • We provide staffing levels that allow our care staff to take the time they need with each patient and family, giving them the personal care and attention they need.
  • The patient to staff ratio at our hospice houses never exceeds more than 3 patients per staff, allowing staff to provide the personal care and attention that each patient needs.

Donations help to support a level of special care and attention that is not required, but we feel is important to the well-being of our patients.

  • We Honor Veterans – is a program that honors patients who served our country. It allows us the opportunity to thank veterans for the service they have given to their country; provide other volunteer veterans to visit with the veteran patient; train staff to understand ways in which the veteran’s service may impact his/her health care needs today; and provide a memorial book for the veteran’s family.
  • Enrichment Services – are services that we make available to our hospice patients that go above and beyond what is required, based on the patients’ needs and preferences. Enrichment Services may include massage therapy, pet therapy, music therapy, and aromatherapy, etc. We also encourage and support celebrations and special events that bring families together.

Donations allow us to provide other services within the community.

  • We host free grief support groups every spring and fall, which are available to anyone in the area who has faced the loss of a loved one.
  • We provide bereavement services for free to anyone who may be struggling with the loss of a loved one.
  • We provide grief support to students in the area when called upon.
  • We provide informational and educational forums throughout the year on issues related to terminal illness, hospice, chronic disease management, palliative care, health care directives, and other related topics. These forums are generally free and open to anyone who would like to participate.

Donations allow us to provide financial assistance to families who are not able to cover the full cost of hospice care, through our Family Assistance Fund.

Since 1984, the support of a generous and caring community has allowed us to honor our commitment to ensuring that patients in our community who need hospice care receive the care they need, regardless of their ability to fully pay for the care they receive from us.

We are also thankful for these local organizations for their support and partnership!
