We offer a homelike environment that enhances the quality of life and maintains respect for elderly persons that can no longer manage their healthcare needs. Daily services and amenities are designed to promote dignity, independence, and wellbeing by improving the quality of life of each person through activities and companionship. Personalized care plans are supervised by the guidance of a licensed nurse and each resident has access to trained caregivers that are always on premises.

 

Enjoy a setting that allows freedom with the comfort of knowing that supporting services are available. Also, enjoy supportive services like housekeeping, three home-style meals per day, personal care, and medication management. The nicely appointed common areas are well suited for group events and gatherings, while social and recreation opportunities create a sense of community. Our facility is a single-story building to avoid confusing and security for those unable to climb stairs. Our facility locks its door at night to provide safety for our residents and nighttime staff.

Resident Living Areas

Each room has a comfortable private living area with a full bath and kitchenette. Each room has individual controlled temperature settings. Each resident is given a personal pendant that can be worn as a necklace or bracelet/watch that is part of our call system. They are instructed to push the pendent when they need assistance and staff will arrive to help. This call system works 24/7.

We offer 4 different sized rooms:

  • Unit A: 260 Sq. feet
  • Unit B: 415 Sq. feet
  • Unit C: 430Sq. feet
  • Unit D: 270 Sq. feet

Weekly light housekeeping (sweeping, mopping, vacuuming, bathroom cleaning, furniture surfaces dusted, but not moved)

The Prairie Home Fieldcrest Difference

At Fieldcrest, we will work with you to establish an individualized service plan that’s right for you. Your service plan will reflect your specific needs and preferences. You will have access to services 24 hours per day, seven days a week. Generally, we will have 2 Aides on-site 24/7; a Licensed Practical Nurse (LPN) on-site during the day M-F and for a few hours on Saturdays and Sundays; and a Registered Nurse (RN) on-site during the week and available/on-call 24/7. Additional staff will prepare and serve meals, provide housekeeping and laundry services, take care of maintenance needs, and coordinate activities for residents. Our volunteers will also be a part of the team that serves Fieldcrest. Each resident will be assigned an RN, who will visit at least monthly to review your plan and ensure that the services you are receiving are meeting your needs. The RN can also communicate with your physician(s) to share information and coordinate your health care needs.

Personal assistance with activities of daily living, such as: eating, bathing, grooming, toileting, dressing, walking and ambulation, bed making

Prairie Home at Fieldcrest Assisted Living Services

Over time, your needs may change… but your home won’t have to. The broad range of services we provide allows you to remain where you are.  As your needs change, we’ll simply adjust your plan to ensure we are still meeting them.  As a Medicare-certified hospice provider, we can provide hospice services to residents, should that ever become a need. Our goal is to help make Fieldcrest your home for as long as you want it to be.
