Job Title: RN Case Manager

Reports To: Clinical Director

Shift: Full-Time

Profile

The RN Case Manager provides comprehensive, patient-centered nursing care for hospice and home care patients. This role leads care coordination, ensures high-quality clinical outcomes, supervises assigned care team members, and maintains compliance with all regulatory and organizational standards. The RN Case Manager serves as the primary clinical point of contact for patients, families, physicians, and the interdisciplinary team.

Essential Duties and Responsibilities

Reporting to the Clinical Director, the RN Case Manager’s primary role is to provide quality care and compassionate service consistently and according to the care plan, providing Clinical Nursing Care.

  • Serve as primary case manager for patients assigned
  • Conduct initial and ongoing physical and psychosocial assessments
  • Develop, implement, evaluate, and update individualized plans of care in collaboration with the Clinical Director, physicians, and interdisciplinary team
  • Coordinate services and initiate referrals to appropriate disciplines and community resources
  • Participate in bi-weekly Interdisciplinary Team (IDT) meetings
  • Communicate changes in patient condition to the Clinical Director and/or Medical Director
  • Ensure continuity of care, including weekend/holiday coordination when applicable
  • Order necessary medical supplies and equipment

Clinical Nursing Care:

  • Provide skilled nursing care in accordance with physician orders and established care plans.
  • Assess and manage pain and symptoms; implement appropriate interventions and educate patients and families.
  • Administer medications safely in accordance with the Seven Rights of Medication Administration.
  • Obtain, verify, and implement physician orders.
  • Monitor lab results and report significant findings.
  • Respond to patients’ physical, emotional, psychosocial, and spiritual needs.
  • Maintain a safe care environment and follow patient safety standards.
  • Support cultural preferences consistent with patient safety and ethical standards.

Patient & Family Support:

  • Educate patients and families regarding diagnosis, medications, symptom management, and care expectations.
  • Provide emotional support and therapeutic presence.
  • Encourage open communication while respecting privacy and individual preferences.
  • Assist with mobility, transfers, repositioning, and prescribed exercises using proper body mechanics and safety equipment.

Compliance, Documentation & Reporting:

  • Document assessments, interventions, and patient progress accurately and timely in the Electronic Medical Record (EMR).
  • Comply with Quality Reporting Standards and all state and federal regulations.
  • Maintain patient and organizational confidentiality.
  • Adhere to agency policies related to clinical care, payroll, billing, and documentation.
  • Maintain current nursing licensure and complete required continuing education (minimum 12 in-service hours annually).

Supervision & Leadership:

  • Supervise and support assigned clinical staff (LPNs, TMAs, CNAs, HHAs, hospice aides, and volunteers as applicable).
  • Provide training, mentorship, and performance feedback.
  • Ensure staff understanding of policies, procedures, and regulatory requirements.
  • Foster a positive, collaborative, and accountable team environment.

Other Responsibilities & Qualifications:

  • Current RN license in good standing.
  • Experience in hospice, home health, or related clinical setting preferred.
  • Strong clinical assessment and critical thinking skills.
  • Effective communication and interpersonal skills.
  • Ability to work independently with strong time management and organizational skills.
  • Ability to meet physical demands of patient care, including lifting and assisting patients (45+ lbs) using proper body mechanics and equipment.

Work Environment & Physical Requirements

  • This role requires moderate to heavy physical activity, including standing, walking, bending, lifting, repositioning, and assisting patients safely. The RN must be able to perform repeated physical tasks using proper lifting techniques to prevent injury.
