Prairie Home Hospice & Community Care

Prairie Home Hospice is a free-standing, non-profit organization established in 1984. As the first hospice in our area, Prairie Home Hospice has long set the standard for end-of-life care in Lyon County and the surrounding area. Today, we continue to lead the way, with innovative programs that honor choices, preserve dignity and address the needs of those we are so privileged to serve. We continue to be the only free-standing, non-profit hospice and home care provider in the area.

While Prairie Home Hospice has proudly served Lyon County with hospice care since 1984, it is no longer the only hospice agency in the area. If you wish to have Prairie Home Hospice & Community Care provide your home care or hospice services, it is essential to clearly communicate your preference to your physician(s), hospital discharge planner, or other relevant professionals. You have a choice, and by law, it must be respected.
