Meet our Clinical Director: Nicole Jacobson, RN
Nicole has been working for Prairie Home Hospice and Community Care since 2013. Nicole is a Registered Nurse and worked directly with patient and family care when she began at PHH. Recently, Nicole accepted the position of Clinical Director at Prairie Home. As Clinical Director, Nicole oversees the clinical operations for both Hospice and Home Care, which includes supervising the clinical team; ensuring that the organization is meeting or exceeding all clinical and regulatory standards; and most importantly, ensuring that all of our patients and families receive the kind of care that they deserve and have come to know and trust from Prairie Home Hospice for the past 34 years.
Nicole resides in Hanley Falls where she raises four beautiful children with her husband, Brandon. No matter what position she takes on Nicole does it with a grace, tenacity, kindness, humor, patience, and dedication that is unprecedented these days. When asked what her favorite part about working for Prairie Home Hospice is Nicole said, “hands down, the best part about this job is being able to help people, every single day, that really need help the most right now. No matter what their needs, backgrounds, circumstances, financial ability, or diagnosis. . . . we get to help them. It’s a very special job and I feel honored to work for Prairie Home Hospice and Community Care.”
